New York Senator Charles Schumer announced on Friday a new grant that will create almost 100 jobs, improve care, reduce costs and provide advanced training.

The over $26,000,000 investment for the Finger Lakes Health Systems Agency will be used to implement a plan designed to revolutionize health care in Rochester.  Developed by the Finger Lakes Health Care Systems Agency through a coalition of the Rochester Innovation Health Collaborative.

The goal is to improve primary care services for area residents and minimize emergency room visits to save the local health system money. It breaks down into three parts:

  • One-third of the money will be used to to hire new care managers for doctors' offices in Rochester and five surrounding counties.
  • One-third will be used to provide assistance for doctors to streamline the way they treat patients.
  • The last third will be used to improve data tracking practices.

The Finger Lakes Health System Agency received the grant specifically to improve primary care and makes savings from the current system, while improving care for Medicare and Medicaid patients.

Over a three-year period, the FLHSA will train 726 health workers and hire 76 health care providers, community-based coordinators, and practice improvement advisers. The effort is expected to reduce health care costs by $48 million over the next three years by improving people's health.
